We may also disclose PHI in response to a subpoena or discovery request, but only after reasonable efforts to notify you or obtain a protective order. Uses and Disclosures Requiring Your AuthorizationCertain information requires your written permission before it can be shared:Psychotherapy notes will not be used or disclosed without your written authorization except in circumstances permitted by law, such as treatment, training, legal defense, compliance investigations, or preventing serious threats.
MarketingWe will not use or disclose your PHI for marketing purposes.Sale of PHIWe will never sell your PHI.
Uses and Disclosures That Do Not Require AuthorizationWe may use or disclose PHI without your permission when allowed or required by law, including:• Reporting suspected child, elder, or dependent adult abuse• Preventing or reducing serious threats to health or safety• Health oversight activities (audits, investigations)• Judicial or administrative proceedings• Law enforcement purposes• Coroners or medical examiners• Research (with required safeguards)• Specialized government functions (military, national security, correctional institutions)• Workers’ compensation compliance• Appointment reminders and information about treatment alternatives or health‑related benefits Uses and Disclosures You May Object To You may choose to allow or restrict sharing your PHI with:• Family members• Friends• Anyone involved in your care or payment for care Your Rights Regarding Your PHIRequest LimitsYou may request restrictions on how your PHI is used or shared.We may deny requests if they would affect your care. Restrictions for Out‑of‑Pocket PaymentsIf you pay for a service in full out‑of‑pocket, you may request that we not disclose that information to your health plan. Confidential CommunicationsYou may request that we contact you in a specific way (e.g., email only, no voicemail). Access to RecordsYou may request an electronic or paper copy of your PHI (excluding psychotherapy notes). Copies will be provided within 30 days of your written request. A reasonable cost‑based fee may apply. Accounting of DisclosuresYou may request a list of disclosures made in the past six years, except those for treatment, payment, or operations. The list will include disclosures made in the past six years unless you request a shorter timeframe. Correct or Update InformationYou may request corrections to your PHI.If denied, we will explain why in writing within 60 days. Copy of This NoticeYou may request a paper or electronic copy of this Notice at any time. Changes to This NoticeWe may update this Notice at any time.
